Our office's payment and insurance policies are simple. Please bring all CURRENT medical insurance and/or Medicare cards at the time of service. Our office will file your insurance and/or Medicare for you as a courtesy. As a patient, you are required to submit any co-pay or deductible at the time of your appointment. You are responsible for any balance that your insurance does not pay for services rendered. If a referral form is required by your insurance company, please bring the form at the time of your appointment. Payment
- Payment is due in full at the time of service unless prior arrangements are made with our office.
- You are responsible for all balances that are not covered by insurance for any services rendered in office or hospital setting.
- It is necessary to make all special arrangements for payment before Dr. Schoenfeld's services are provided.

Insurance
- Be sure to bring your insurance card to your appointment.
- Our office will file your insurance claim as a courtesy.
- You are required to submit the deductible or co-pay amount at the time of your appointment.
- It is a patient's responsibility to provide a referral form if necessary for your insurance company.
- Our office accepts Medicare assignment. You will be responsible for allowed charges for which Medicare does not pay. Our office will file the Medicare claim form for you.
- Most commercial insurances accepted.
- SELF-PAY - Please contact our office if you do not have any insurance to discuss a payment option.
